We C.A.R.E. Home Health Agency Celebrates Nearly a Decade of Service and Sets Sights on National and Global Expansion

We C.A.R.E. Home Health Agency, a family-owned and community-rooted healthcare provider based in Pennsylvania, announce its upcoming 10th anniversary. As the organization reflects on nearly a decade of delivering compassionate, patient-centered care, it shares its plans to expand its footprint across Pennsylvania, into neighboring states, and ultimately to international markets.

The agency is exploring local and global growth opportunities, driven by the belief that its model of care can transform patient outcomes worldwide. “When I think that all of this started with a dream, my family’s belief in me, and my desire to care for people the way they deserve, it’s overwhelming in the best way,” says founder and CEO Shireeta Benjamin-Cosom. “It’s not just the agency that’s growing. Our purpose is also growing. And we’re just getting started.”

We C.A.R.E. began as a vision sparked by an unexpected opportunity. At the time, Benjamin-Cosom was an experienced ER trauma nurse with an extensive background in emergency medicine, geriatrics, psychiatry, and pediatrics. After a message from a stranger offering her a role in a budding home health startup, she took a detour from her travel nursing career and found herself helping draft clinical policies. When the initial plan fell through, her then-boyfriend (now husband) asked a pivotal question: “Why not us?” That challenge planted the seed for We C.A.R.E.

Benjamin-Cosom approached the venture as a nurse. She had seen firsthand in the ER that patients needed to be treated not just as medical cases but as people in need of care. Her values became the blueprint for We C.A.R.E.’s ethos. “We built the agency on a promise to care for patients with compassion, respect, and dignity, while also valuing the staff,” Benjamin-Cosom shares.

Today, We C.A.R.E. offers a full continuum of home-based services designed to keep patients out of hospitals and in the comfort of their homes. It currently serves 13 counties and provides services such as skilled nursing, therapy, medical social work, home infusion, non-emergency medical transportation, meal delivery, and personal emergency response systems. Its team includes certified home health aides, speech and occupational therapists, physical therapists, and respiratory care specialists.

Besides its services, the agency stands out for its philosophy. From administering wound care to preparing hot meals for seniors, every service is part of a larger goal of helping patients live well, independently, and with dignity. Moreover, We C.A.R.E. is committed to ensuring equity in care, particularly for patients with chronic conditions, disabilities, and those recovering from surgery.

Benjamin-Cosom’s nurturing vision shaped the agency’s mission and model. She recognized early on that home-based care could and should prevent unnecessary hospital readmissions, reduce healthcare costs, and offer a better quality of life. This approach is noteworthy, especially in Pennsylvania, where she hopes to formally establish a new standard of home-based healthcare that aligns with the emerging “hospital-at-home” model.

As We C.A.R.E. Home Health Agency prepares for its 10th anniversary, it remains dedicated to reimagining healthcare at home. Its vision is to become a one-stop shop for all home-based health needs locally, nationally, and globally.
