Where AI Breaks Inside Organizations

• Capital is dissipated: Nearly 90 percent of organizations report using AI in some form, yet fewer than 40 percent see enterprise-level financial impact. 
• Authority lags conditions: In higher-maturity environments, strategic decisions compress to days. Yet authority often remains trapped in monthly committees and quarterly rituals. Decisions are approved intact and arrive irrelevant.
• Autonomy is constrained by avoidance: AI systems already execute routine operational decisions with high accuracy. They remain advisory not because they are untrusted, but because leadership has not settled its tolerance for loss. Ambiguity becomes a substitute for governance.
